Does Urinary Vitamin D-Binding Protein Have a Role in the Prediction of Steroid Resistance in Nephrotic Syndrome? A Cohort Study on Egyptian Children

Background: Nephrotic syndrome (NS) is a common glomerular disease in children, for which there are currently no noninvasive markers predicting responsiveness to steroid treatment. Objectives: This study aimed analyze the relationship between clinical-laboratory presentations and subsequent response therapy evaluate role of urinary vitamin D binding protein (uVDBP) as predictor resistance (SR). Methods: was prospective longitudinal on 60 children with active NS (30 patients newly diagnosed 30 relapsed NS). In addition, control subjects served reference group uVDBP values. The were evaluated before starting therapy, monitored 4 - 8 weeks later. Results: levels elevated (33.33 ± 15.6 ng/mL) compared (17.35 5.5 ng/mL, P < 0.001). Also, significantly higher (37.93 16.2 than (28.73 13.67 = 0.014). level presentation did not differ who had steroid-sensitive nephrotic (SSNS) (34.91 15.01 steroid-resistant (SRNS) after treatment (32.60 15.94 0.417). Steroid associated younger age onset, specifically below 2.1 years (P 0.023), longer duration illness 0.007), having more relapses 0.002), total leucocytic count > 8.05 × 103/mm3 0.031), platelet counts 516.5 0.044). Conclusions: this study, we found that could reflect severity rather predict pattern responsiveness. Younger illness, previous relapses, increased leukocyte SRNS.
